Protocol Questionnaire

[FrontPage Save Results Component]
In order to best serve your personal healing needs we would appreciate your completion of the below questionnaire. Our objective is to create a national data base of men like yourself that are taking proactive responsibility for their health. The information you supply will not only help us serve your needs more appropriately, but will also help those men that contact PAF looking for others like themselves from their geographic area.
Many of the inquires we receive at PAF are from men all over the country. They want to know, just as you do about others on successful protocols, supplements, and dietary measures. They want to be aware of positive as well as negative experiences with health professionals and supplementation.
Some of these questions are of a sensitive nature. Your input is appreciated. PAF is composed of men with prostate cancer, sharing concerns, and offering hope for a positive healing experience.
Please indicate if other men may call you about your personal experience by checking this checkbox .
Upon completion please click 'submit' button to e-mail the questionnaire to
kamalik@sbcglobal.net, or you can print it out and fax it to 415-661-2691, or mail it to:
